Job Summary:
The Skills Trainer Coordinator provides general support to Adult Day Vocational Program (ADVP) Director. Responsible for monitoring the safety and well-being of program participants while assisting in the development and delivery of the following: health and wellness, social/recreational activities, integrated community activities, volunteer opportunities and independence skills programming. Job may include personal care support, work production support, respite relief, and other duties required to provide a quality rehabilitation program to those receiving services either onsite, at their home, or in the community. Reports directly to the Adult Day Vocational Program (ADVP) Director.
Primary Duties

Provide daily health and wellness, social/recreational activities, community/volunteer opportunities, and independence skills programming and supervision of program participants ensuring their health, welfare and needs are met while following program guidelines.
Assist with rehabilitative program implementation. Ensure familiarity with goals as indicated on participant's Individual Support Plan.
Adhere to regulatory requirements and ServiceSource policies and procedures to ensure compliant documentation for participants. Assist with quarterly progress notes, accurate documentation of daily progress notes, incident reports (when applicable), and other data as needed.
Monitor meals, breaks, work, and recreation areas to ensure the constant safety and whereabouts of program participants.
Additional Responsibilities
Assist participants with meals, personal care, and community integration as needed.
Provide crisis intervention support as needed. Report every incident to the ADVP Director immediately.
Complete incident reports within 24 hours. Ensure ongoing compliance with Human Rights, Medicaid, CARF, and funding source requirements and regulations.
Assist ADVP Director with the implementation of program functions including, but not limited to training classes, downtime activities, group trips and recreational activities.
Monitor and assist with the boarding and exiting of participants from their mode of transportation.
Implement and monitor program goals as directed by ADVP Director. Provide ideas for goals, positive behavior approaches/interventions, personal enrichment, and/or social adjustment. SERVICE CAPACITY - includes, but is not limited to the following: ADVP, Supported Employment, Long Term Supports, NC Innovations Waiver 1915 (c) Day Supports, NC Innovations Waiver 1915 (c) Community Networking, NC Innovations Waiver 1915 (c) Community Living and Supports, NC Innovations Waiver 1915 (c) Supported Employment,NC Innovations Waiver 1915 (c) Respite.
Perform other responsibilities as assigned.
Qualifications: Education, Experience and Certification(s)
High school diploma or General Educational Development (GED), plus 1-year related experience working with people with disabilities required.
Experience in providing direct programming to people with disabilities preferred.
Valid North Carolina driver license, per state law, and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).
Must pass a check of the NC Healthcare Personnel Registry.
Knowledge, Skills and Abilities
Ability to communicate effectively in oral and written form.
Ability to establish and maintain effective working relationships with program participants, families, all levels of staff, and community agencies.
Ability to read and interpret documents such as safety rules, operating/maintenance instructions, and procedure manuals.
Ability to operate a computer using Windows. Knowledge of Microsoft Office is preferred, particularly: Access, Word, and Outlook.
Basic knowledge of principles and techniques of working with people with developmental disabilities is preferred.
Ability to relate to people at all levels of the organization.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standard exists.
