$55/hour! Multi-Site RN Case Manager M-F 1st Shift - Delta-T Group Inc.

One of our clients is seeking a Multi-Site Servicing RN Case Manager at an IDD Facility located in the Northeast Philadelphia area.

CLIENT'S AVAILABLE HOURS
8:30am until 5pm, Monday through Friday

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* In this role, you will have the opportunity to provide case management support to individuals in group homes, while working closely with the patient care team via zoom and in-person meetings.
* Provide clinical assessment, and medication administration to patients.
* Go to group homes to deliver medication, update patient treatment logs, and medical charts.
* Partner with healthcare professionals to coordinate patient care.
* Perform other duties as assigned.

CLIENT'S REQUIRED SKILLS & EXPERIENCE
* Graduate from an accredited Nursing Program.
* Current RN license in the state of PA.
* Possession of a current and valid PA driver license.
* Previous experience and/or interest working with individuals with physical and developmental disabilities.
* Ability to perform professional nursing duties, including but not limited to assessments, treatment, case management, etc.
* Experience with outlook, MS Office, MS Word is REQUIRED!
