Position Definition:
Provide activities per the Monthly Activity calendar. Responsible for performing direct care duties and assist in maintaining a positive physical, social and psychological environment for patient population. Assist Activity/Life Enrichment Director in maintaining department in compliance with federal, State and local regulations.
Essential Duties:
• Assist in implementation of approved activity program based on the concept of encouraging residents to reach their highest personal, spiritual, and psychosocial potential through meaningful engagement
• Assist in maintenance and expansion of each resident’s intellectual capacity, creativity, and life enjoyment
• Perform facility rounds to provide assurance that residents have some form of stimulation provided.
• Perform resident screenings/data gathering using minimum data set (MDS) instrument as requested by Activity/Life Enrichment director. Complete resident assessment protocols including RAI/RAPS as directed.
• Follow activity program to ensure residents become meaningfully engaged throughout each day and evening
• Distribute current monthly activity schedules and calendars.
• Provide craft supplies, materials, library services, tapes, records, and Braille reading materials and assist residents in activities of choice.
• Assist Activity/Life Enrichment director to ensure that all necessary forms, reports, and progress notes are properly completed in an informative and timely manner.
• Participate in implementation of activities programs and discharge planning to meet facility needs.
• Conduct activities to ensure that residents’ social and emotional needs are met
• Report change in residents’ conditions to Activity/Life Enrichment director and nursing personnel.
• Provide feedback to Activity/Life Enrichment director for activities material, equipment, and supply needs.
• Assist Activity/Life Enrichment director to maintain facility decorations for each Holiday and special events at least three weeks before event and ensure the decorations are consistent, and timely with the current Holiday/event.
• Assist residents with all basic functions including bathing, dressing, grooming, dental care, bowel and bladder functions, preparation for medical tests and exams, ear and eye care, positioning, etc.
• Provide nursing support as directed by Director of Activities/Life Enrichment, Licensed Nurse and/or DON.
• Record all necessary charting entries.
• Prepare residents for meals and assist with dining as needed, including offering substitutions when appropriate.
• Perform other related duties as assigned by Activity Director/Life Enrichment.
Minimum Requirements:
• Must be a Certified Nursing Assistant in good standing and meet all applicable federal and state licensure requirements or in training to satisfactorily complete the requirements to become a Certified Nursing Assistant.
• Must be knowledgeable of activities within long-term care environment.
• Good interpersonal and written/oral communication skills
• Strong organizational skills
• Skill in accuracy and attention to detail
• Ability to manage multiple projects, tasks and priorities to achieve desired goals in a timely manner
• Maintain confidentiality and integrity of position
• Follow company policies and procedures, including but not limited to good work habits and attendance practices
