Job Summary
The Coordinator serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information. Responsible for identifying high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, counseling, care management, and referrals to appropriate resources. Works collaboratively with the addiction care team to ensure patient needs are met and care delivery is coordinated across the continuum.
Job Duties
+ Works in conjunction with physicians, nurses, addiction specialists and others in the design, implementation, and evaluation of the addiction medicine care model.
+ Identifies the targeted population within his and her practice site and risk stratifies all members to prioritize needs and direct interventions.
+ Develops and implements appropriate treatment plans and interventions.
+ Completes appropriate Federal, State and Local Forms.
+ Initiates and monitors completeness, accuracy and timeliness of all documentation, as required.
+ Coordinates care with appropriate resources and performs and facilitates home visits with the Community Health Assistant, in collaboration with the PCP, PA Single County Authorities, and other appropriate community resources.
+ The Coordinator may visit the patient's home, attend primary care visits, specialty visits or alternate level of care, as appropriate, in the management and coordination of complex substance use disorder patients.
+ Documents in appropriate electronic system by the close of business each day.
+ Consults with Attending physician and physician representative or Advisor and Medical Director and leadership as necessary to resolve barriers through appropriate administrative and medical channels.
+ Works with Addiction Services Leadership to continuously evaluate process, identify problems, and propose process improvement strategies to enhance the care delivery model.
+ Participates in on call systems, per policy or per program designation, priorities and adjusts workload priorities based on feedback from physicians and immediate supervisor.
