Mayfield Heights Nursing and Rehabilitation is a 150-bed facility providing long-term care, skilled rehabilitation, intensive outpatient care, and memory care. We are currently seeking a full-time Admissions Director to join our team. The ideal candidate must be devoted and passionate about their chosen career. Choosing to work at Mayfield Heights Nursing and Rehabilitation is a decision you’ll be happy you made!We are located at 6757 Mayfield Rd., Mayfield Hts., OHResponsibilities:Establish and maintain positive relationships with referral sources to promote skilled servicesCoordinate the referral process from the identification of the appropriate payer source to the analysis of patient informationResponsible for achieving the proposed company census and revenue objectivesCollaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members, and social work staff members to ensure seamless transfer to the facilityCommunicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely mannerAnalyze patient needs and match with facility services to plan for an appropriate facility placementOther duties as assigned by supervisorQualifications:Prior experience in a long-term care setting preferredLicensed Practical Nurse (LPN) or Registered Nurse (RN) qualifications optionalMinimum of two (2) years experience in long-term careProven ability to successfully market the services of the organization.If you are a positive thinker and possess a "can-do" attitude we invite you to come join our team!We are an Equal Opportunity Employer and promote a drug-free workplace. Responsibilities:

- Responsible for achieving the proposed company census and revenue objectives

Requirements/Qualifications:
- Prior experience in a long-term care setting preferred
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) qualifications optional
- Minimum of two (2) years experience in long-term care
- Proven ability to successfully market the services of the organization.
