Shift: Shifts Vary (9:00a-5:30p, 10:00a-6:30p, 11:00a-7:30p), Monday through Friday with On-call rotation

What will I do as an employee with Redstone?
As an RN/LPN Admissions Nurse, supporting the Redstone mission will include the following responsibilities:
- Primarily responsible for the introduction of home health services, reflective of regulatory requirements and quality standards. Provides nursing care to clients in a variety of settings. Responsible for the implementation of services to meet client needs. Follows current nursing professional standards of practice and agency approved protocols for home health.
- Provides initial, and when needed, ongoing assessment of clients, assuring an accurate evaluation of client needs. Identifies when clients need, and qualify for, additional services and makes referral to appropriate Redstone@Home manager or other community resource. Assesses client and caregiver needs, provides client and caregiver education. Educates clients and caregivers on the implementation of technology for the purpose of communication and virtual care.
- Maintains current and accurate records through use of the current EMR and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines.
- Collaborates with Interdisciplinary team members and attends required meetings. Performs supervisory visits of Home Health Aides, and/or Licensed Practical Nurses, as needed, assuring the delivery of quality health care services per program policies.
- Completes home visits and on-call duties after normal business hours, on weekends, and holidays as needed or assigned. Conducts wellness visits as assigned.
- Verifies client identify via photo identification, insurance verification, and/or physical recognition. Secures reliable consent, in writing, for all ordered home health services. When unable to obtain written consent, the admission nurse will obtain verbal consent for care from the patient, POA, or next of kin.
- The admission nurse performs the initial client assessment, determining eligibility for home health care, based on CMS/CGS regulatory guidelines and company policy.
What do I need for this role with Redstone?
- Requires a license in the state of Pennsylvania as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Two years (2) of experience in hospice is preferred.
