Advance Care Planning Facilitator - Cottage Health

Provides advanced care planning facilitation with patients and families, with focus on Spanish speaking population. Preferred social work, spiritual care, case manager, or community health worker experience.

Bachelor's degree required and one year of psychosocial critical care work and experience with end of life discussions and advanced care planning.

Must have 1 year psychosocial critical care work, experience with end of life discussions and advance care planning. Fluent in Spanish.
