Full Time Aftercare Coordinator Mount Sinai Wellness Center
This position providescare coordination services to patients participating in treatment at Mount Sinai Wellness Center.TheCare Coordinatorprovidesa link between the patient, client support system, community, and other healthcare and social service professionals.The Care Coordinatoraddressesany barriers to care and provides referrals for needs that include housing, medical and continued mental health services. In addition, they conduct outreach to engage community residents and to improve care coordination.This position will also act as an advocate for the client with other serviceproviders when appropriate to do so.
Job Qualifications:
- Possession of aBachelors Degreewith minimum of two yearswork related experience in mental health field engaged in case management, mental health, aftercare, substance abuse or related areas.
- Excellent written and oral communication skills
- Demonstrate strong critical thinking, problem-solving and organizational and time management skills.
- Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors.
- MS Office proficiency.
- Abilityto pass all pre-employment testing,including, butnot limited to,UDS clearance andbackground check.
Essential Functions:
- Manages a caseload of patients who are receiving treatment at Mount Sinai Wellness Center.
- Documents contact withpatients in order to assess needs and maintain required paperwork and engage family members,in accordance withMount Sinai Wellness Center, The Joint Commission, and state regulations.
- Conduct outreach and facilitate access to healthcare services.
- Ability to demonstrate a high degree of flexibility and competence in adjusting to patients with varying degrees of traumatic experiences and complex psychological presentations.
- Ability to address a variety of diversespiritual, emotional, and social needs in multiple environments of an often-high-risk population, using a trauma-informed care approach while maintaining a highdegree of professionalism and cultural awareness.
- Possess the ability to work with a wide range of patients including adultsand families.
- Collaborates with community agencies, families and other natural support systems to ensure patients needs are met.
- Develops a strong working relationship withcommunity partners.
- Participates inmulti-disciplinarymeetings toadvocate for patients and community to coordinateconsistent and timely provision of services.
- Ability to facilitate referral process to assure continuity of care.
- Ability to maintain client records in accordance with Mount Sinai Wellness Center, HIPAA, TJC, Federal, State, and local requirements.
- Participation in all required Mount Sinai Wellness Center training.
- Performs other duties as assigned by supervisor.
Salary Range:
Generally starting at $21.15 an hour.
