Fellowship House is currently seeking a bachelors-level clinician with at least one year of experience working with adults with mental illness to join our Florida Assertive Community Treatment (FACT) Team as a Case Manager. Our internationally accredited FACT Team consists of a multidisciplinary group of mental health professionals who collaborate to deliver comprehensive treatment, rehabilitation, and support services - empowering it's members to achieve their goals. As a Case Manager, you'll assist program members in providing supportive counseling, establishing a supportive social network, and developing rehabilitation goals and achievements for recovery.
Key Responsibilities:
- Provide individual support to our FACT members, promoting growth and personal development.
- Maintain positive relationships with FACT members and provide supportive counseling/therapy in the home and out in the community.
- Support in the development of rehabilitation goals and achievement of recovery.
- Help FACT Members in achieving their goals, learning symptom management, and developing positive coping skills.
Requirements:
- Bachelor's degree in Human Services field (social work, psychology, sociology, criminal justice, health education, etc.) (REQUIRED)
- A minimum one year experience working with the adult mentally ill population
