Job Description
Sonoma Valley Community Health Center is seeking a full-time Behavior Health Clinician (PhD, PsyD or LCSW) who is licensed in California. The Behavior Health Clinician, in collaboration with primary care providers, will develop and deliver behavioral health treatment services in an integrated healthcare model.
This position will be responsible for applying social work, case management and counseling in collaboration with medical and paramedical professionals, in the assessment and treatment of patients. Provides consultation services to include brief assessment, crisis intervention, case management and referrals, coping skills training, psycho-education, and support of medical treatment regimen. Provides liaison services to include formal and informal education of providers and Health Center staff of mental health services, psychological disorders, and empirically validated treatments. Maintain a caseload of ongoing therapy patients, using the cognitive brief therapy model.
Qualified candidates will meet the following criteria:
* Master’s degree from an accredited educational institution in social work, psychology, or related human services field of study.
* Must have a valid, unencumbered California license.
* Certification by a national behavioral health association preferred.
* Minimum 2 years direct practice experience within a mental health setting, preferably with severe and persistently mentally ill adults.
* Knowledge of the special needs and behavioral characteristics of the severe and persistently mentally ill adults.
* Knowledge of case management principles.
* Ability to effectively use CPI and behavior management techniques.
* Strong interpersonal and organizational skills.
* Excellent written and verbal communication skills.
* Computer literacy in word processing and spreadsheet programs. Computer literacy using an electronic health records is a plus.
* Ability to handle multiple tasks with creativity, flexibility, and energy.
* Focus on adolescent and woman’s health issues a plus.
* Bilingual in English and Spanish is desirable.

Our Health Center provides medical care and preventative services to people of all ages: newborns, children, teenagers, childbearing women, adults and the elderly. We are the only “safety net” provider serving the residents of the Sonoma Valley and the only primary care provider in Sonoma Valley currently accepting Medi-Cal and uninsured patients.
