Job Summary
We are seeking an experienced Behavioral Health Specialist dedicated to providing comprehensive care within an interdisciplinary team, working specifically with vulnerable senior populations. This role collaborates with the Social Work team and reports to the Social Work Manager, following the PACE (Program of All-Inclusive Care for the Elderly) model to address participants' behavioral health needs. You will manage a caseload, offering individual and group therapy, crisis intervention, and care management to support holistic health outcomes.

Key Responsibilities

• Conduct comprehensive behavioral health assessments to identify care needs and goals.
• Participate in interdisciplinary team meetings, contributing to tailored care plans with discipline-specific goals.
• Implement and document behavioral health interventions, including counseling, symptom management, and facilitating care transitions.
• Lead individual and group therapy sessions, such as art therapy, music therapy, and structured discussions to enhance participants’ social roles.
• Provide training to staff on behavioral health symptoms, treatments, and intervention strategies.

• Master's degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD).
• Licensed by CA Board of Behavioral Sciences as an LCSW or Licensed by the CA Board of Psychologists as a Psychologist
• Minimum five years of experience in behavioral health, including therapy, assessments, and care planning needs.
• At least two years working with elderly or frail populations.
