Description: Care Navigator / Med Advisor



+ High volume outbound calls, making 40 calls/day.


+ Conducting telephonic medication adherence outreach calls.


+ Uses motivational interviewing to conduct the telephonic medication adherence outreach call.


+ Active listening, asking probing questions, critical thinking.


+ They will recommend solutions, and complete interventions as directed by the workflows and/or training materials.


+ Call pharmacies and providers if needed for follow-up.
