The Care Coordinator supports our mission and values by providing support to the residents of RSL through admissions coordination and care management. This position also maintains census in the building by responding to inquiries and providing tours.
What will I do as a Care Coordinator?
Housing
- Serve as grievance coordinator for Riverside Lodge.
- Actively identify and respond to resident questions and concerns in a timely manner, partnering with Executive Director (ED).
- Complete required documentation - resident's rights, grievance procedure as designated in each setting.
- Participate in Leading Age collaborative meetings for Assisted Living.
- Process resident agreement and related paperwork, including applications and contracts, financials, and other information as required.
- Oversee new studio suite readiness.
- Assist with the coordination of resident moves across the continuum.
- Manage and conduct new resident orientation.
Admissions
- Responsible for lead generation to ensure census goals are achieved.
- Responsible for lead management, prospecting, and follow up to include:
- Initial screenings
- Tours
- Follow up calls
- Finalize sale
- Use internal software for lead management.
- Coordinate new resident resources to facilitate move-ins.
- Responsible for new resident application, financial process, and final contract signing of admissions agreement.
- Participate in campus and community events to raise organizational awareness.
- Manage electronic medical record (EMR) patient profiles.
- Serve as back up for Care Navigator-SNF/Transitions Coordinator-SNF
- Provide input regarding AL marketing materials and manage AL collateral material.
- Plan, coordinate, and participate in various campus and community events and community outreach opportunities.
Care and Discharge Planning in Partnership with Nursing Team
- Educate residents and families on Advance Directives.
- Direct residents/families to appropriate agencies should legal problems arise that cannot be met within the facility.
- Educate residents/families on available financial resources - both public and private programs.
- Direct residents/families to appropriate agencies for assistance that cannot be met within the facility.
- Assist with orientation of new residents.
- Coordinate resident needs with families, friends, and facility staff by acting as liaison between them to ensure good communication for the welfare of the resident.
- Provide brief counseling interventions in resident crisis situations.
What is required in the Care Coordinator position?
- Bachelor's degree in marketing, social services, or a healthcare related field preferred.
- Minimum of 1 year in sales in a health care industry preferred.
- Minimum of 1 year in admissions/care coordination in a health care industry preferred.
- Proficient in written and verbal English and able to make sales presentations. Goal oriented, strategic thinker, result focused.
- Ability to handle hostile or emotionally upset residents and families.
- Microsoft Office skills.
