Position: Care Coordinator
Job Location: Vista
Hourly Range: $32.00 - $42.00 per hour (based on experience)
Role Overview:
The Care Coordinator is integral to our medical office, acting as a liaison between patients, healthcare providers, and insurance companies, ensuring a seamless patient experience and efficient healthcare delivery.
Essential Duties
- Coordinate and schedule patient appointments, procedures, and follow-ups.
- Manage patient records with accuracy and confidentiality.
- Facilitate communication between patients, medical staff, and external agencies.
- Guide patients in understanding healthcare plans and treatment options.
- Handle insurance authorizations, claims, and patient eligibility verification.
- Educate patients about preventive care and wellness.
- Monitor and track patient outcomes and follow-up requirements.
- Resolve patient concerns related to care coordination.
Qualifications:
- High School diploma or equivalent
- Medical Assistant Certificate or CPR Certification preferred.
- Previous experience in a healthcare setting.
- Proficiency in healthcare software and electronic medical records.
- Knowledge of medical terminology and insurance processes.
