Job Title: Care Navigator – Hybrid Role (Remote/Home-Based with Facility Visits)
Location: Must reside in Honolulu, HI

By combining state-of-the-art technology with personalized support, we help keep patients on the path to recovery, avoiding unnecessary hospital readmissions. In collaboration with health systems and physicians, we support patients from discharge to home, aiming to reduce readmissions and improve outcomes. We are seeking a clinically trained professional comfortable working with patients discharged from skilled nursing facilities.

Key Responsibilities:

• Conduct bedside consultations and secure consent for program participation with patients at local facilities, on a daily or weekly basis as required.
• Participate in SNF (Skilled Nursing Facility) team meetings to discuss discharge planning.
• Collaborate closely with SNF discharge planners and team members to coordinate smooth transitions from facility to home.
• Set up patients with remote monitoring devices as needed.
• Coordinate care in partnership with the client's team.
• Manage multiple patient transitions efficiently.
• Document interactions thoroughly within designated software platforms.

Requirements:

• Active LPN Nursing license OR Medical Assistant (MA) certification in Hawaii.
• Minimum of 1 year of patient care experience.
• Preferred: 1 year of home care experience (not required).
• Reliable transportation and willingness to travel as necessary.
• Strong team player who can adapt to workflow changes.
• Capable of working both independently and as part of a team.
• Ability to commit to a full-time schedule.
• Home internet access required.
• Proficient in technology; able to quickly learn software and devices.
• Excellent customer service skills.
• Familiarity with Remote Patient Monitoring programs is a plus.
• Experience with Microsoft Office (Outlook 365, Teams, Excel, Word) and EHR systems.
• Typing speed of 50 WPM or higher.

Days/Hours:

• Full-Time position based in Honolulu.

Pay:

• Salary based on experience.
• Mileage reimbursement provided.
