Job Description: Vitability Health is seeking dedicated and certified Community Health Workers to join our team as Care Navigators. In this role, you will support our Chronic Care Managers (RN) by assisting patients in accessing social services and community resources. Your work will be vital in ensuring our patients receive comprehensive care that addresses their social, medical, and health needs within the New Jersey healthcare system.
Key Responsibilities:
- Assist patients in navigating the healthcare system and accessing community resources and social services.
- Collaborate with Chronic Care Managers (RN) to identify patient needs and develop action plans.
- Coordinate and facilitate connections to community resources such as housing, transportation, food assistance, and other support services.
- Conduct home visits and telephonic outreach to assess patient needs and provide support.
- Educate patients and their families on available resources and how to utilize them.
- Maintain accurate and up-to-date patient records and documentation in accordance with HIPAA regulations.
- Advocate for patients' needs within the healthcare system and community.
- Work closely with multidisciplinary teams to ensure continuity of care and comprehensive service delivery.
- Stay informed about medical resources and healthcare services available in New Jersey.
- Assist patients in understanding their medical conditions and navigating their healthcare plans.
- Participate in community outreach and education initiatives to promote health and wellness.
Qualifications:
- Certification as a Community Health Worker (CHW) is required.
- Experience in a healthcare or social services setting is preferred.
- Strong understanding of community resources, social services, and medical resources available to patients in New Jersey.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in using electronic health records (EHR) and other relevant software.
- Compassionate and patient-centered approach to care.
