POSITION SUMMARY: The Care Transitions Manager coordinates the day-to-day operations of the facility and works closely with the Behavioral Health Director to provide overall support and oversight to the CP Care Transitions program. This position is responsible for staffing, performance management and identifies opportunities for ongoing education. Responsible for implementing and supervising nursing services in the medical detoxification and residential program and ensuring the programs meet regulatory and CPH policy requirements. Meets patients where they are at in system of care and facilitates referral to withdrawal management facility. Applies review criteria to determine medical necessity for admission and continued stay; provides clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost-effective quality healthcare.
REQUIREMENTS:
- Associates degree in Nursing
- Current Alaska Nursing License
- Four years of experience of addiction treatment experience
- One year of leadership experience
It is required that individuals in this position have, or obtain upon hire, a National Provider Identifier number (NPI), assistance provided. Upon hire, with the assistance of the Patient Financial Services department, they must enroll in Alaska Medicaid as an 1115 Waiver Provider and maintain active enrollment, as required for services to be paid by Alaska Medicaid.
