Case Manager – Safe Haven - Deborah's Place

Position Summary
Dolores' Safe Haven provides flexible, low demand housing for residents with complex mental health diagnoses and many barriers to housing. This position is responsible for providing direct clinical and case management services to this population, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.
Essential Duties and Responsibilities
Case Management
- Assess participant physical, emotional, and financial needs through intake and on-going individual meetings
- Engage tenants daily in the program milieu, providing daily support
- Utilize motivational interviewing techniques to determine participants' readiness to change and plan appropriate interventions
- Assist participants with prioritizing self-identified goals and planning to achieve those goals.
- Create ongoing therapeutic groups and activities to enhance resident's life skills and encourage community participation
- Evaluate progress related to goals
- Provide necessary resources to participants
- Provide support and guidance to participants through the process of goal-achievement.
- Connect participants with appropriate referrals to meet their self-identified needs
- When appropriate, assist tenants in moving on to other housing in the community by identifying housing resources and assisting tenants in completing housing applications and moves to other housing units, either in or outside of Deborah's Place.
- Work collaboratively with the Residential Manager and staff regarding participant concerns via staff meetings, program log, and written updates
- Provide crisis intervention when necessary
File/Record-keeping and Data Quality
- Keep individual participant files updated and orderly, including progress notes, intake and needs assessment, service plans, releases of information and referral forms
- Maintain daily, weekly and monthly service statistics and enter this data in the HMIS database
- Work collaboratively with the Program Data Manager to ensure data quality
Other
- Maintain required professional knowledge and job skills. Attend and participate in required educational programs and staff meetings
- Act as a member of the Clinical Services Department and participate by sharing knowledge, attending Clinical Team meetings, and completing clinical staffing
- Represent Deborah's Place mission and values to participants, tenants, visitors, volunteers and others
- Perform other duties as assigned
Qualifications and Requirements
Education: Bachelor's Degree in Social Work or related field. Master's degree preferred.
Licensure and/or Certification: Professional Licensure as a Licensed Clinical Social Worker (LCSW) in the State of Illinois preferred or Licensed Social Worker (LSW) preferred
Job Experience: 2-3 years of case management or social service experience
Special Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, effective oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and own vehicle preferred.
