Position Summary: The Case Manager plays a pivotal role in supporting clients through their recovery journey. This position involves comprehensive case management, including assessment, planning, implementation, and evaluation of client care. The Case Manager will work closely with clients, their families, and a multidisciplinary team to ensure coordinated, client-centered care that promotes long-term recovery from substance use disorders.
Key Responsibilities:

Client Assessment and Intake:
- Conduct thorough assessments of clients' physical, emotional, social, and educational needs.
- Develop individualized treatment plans in collaboration with clients, families, and the treatment team.

Treatment Planning and Coordination:
- Coordinate and facilitate access to services, including medical care, therapy, housing, employment, and educational resources.
- Monitor and adjust treatment plans as needed to reflect clients' progress and changing needs.
- Ensure that clients are engaged in appropriate levels of care throughout their treatment.

Client Advocacy:
- Advocate for clients' needs within the treatment center and in the community.
- Assist clients in navigating healthcare and social service systems.
- Empower clients to utilize available resources and support networks effectively.

Documentation and Reporting:
- Maintain accurate and up-to-date records of client interactions, treatment plans, and progress notes.
- Prepare reports for case reviews, funding sources, and regulatory agencies as required.
- Ensure compliance with all relevant regulations, standards, and policies.

Collaboration and Communication:
- Work closely with a multidisciplinary team, including medical professionals, therapists, and peer support specialists, to deliver integrated care.
- Facilitate regular case conferences and team meetings to discuss client progress and barriers.
- Communicate effectively with clients, families, and external partners.

Crisis Intervention:
- Provide immediate support and intervention in crisis situations.
- Develop and implement crisis management plans to ensure client safety and stability.

Education and Support:
- Educate clients and their families about substance use disorders, treatment options, and recovery processes.
- Facilitate support groups, workshops, and educational sessions.
- Promote a positive and supportive environment that encourages client engagement and recovery.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Master's preferred).
- Certified Case Manager (CCM) or equivalent certification preferred.
- Minimum of 2 years of experience in case management, preferably in a substance use disorder treatment setting.
- Strong understanding of substance use disorders, treatment modalities, and recovery resources.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, non-judgmental approach to client care.
- Knowledge of local, state, and federal regulations related to substance use disorder treatment.
- Proficiency in electronic health records and case management software.
