Position Title: Case Manager CRT Job Location: Bennington, VT Education Level High School Salary Range: $21.85 - $21.85 Hourly Job Shift: Day Job Category: Non-Credentialed Position Description: OBJECTIVES / PURPOSE: Under the direction of the Outpatient/ CRT Community Service Manager, the Case Manager implements, supports, and helps coordinate the services for chronically mentally ill adults living in the community in conjunction with their individual treatment plans. MAJOR RESPONSIBILITIES: 1.0 CLIENT SUPPORT: Provides case management, service coordination, supportive counseling, and skill building for assigned clients. Supports daily living skills, shopping, budgeting, and coordinates services for meeting housing needs for clients. Provides assistance in getting income support and entitlement. Helps to foster peer supports. Facilitates growth of community and family support. Facilitates assistance to meet necessary mental and physical health needs. Participates in all aspects of CRT. If or as assigned to provide Substance Use services, or within related SUD grants, the incumbent will show competencies in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories, in order to provide effective care. 2.0 CLIENT IDENTIFICATION AND OUTREACH: Provides outreach services to open clients of the community who are in need of our services. Develops systems to provide services to area clients. Provides protection and advocacy of client's rights whenever possible. Provides clients with referral information and assists with transportation as needed. Provides support to group home clients as needed. 3.0 INFORMATION MANAGEMENT AND RECORD KEEPING: Completes various clinical and administrative forms for billing, information, and clinical records. Completes all case management documentation within 24 hours and finalized within 3 business days and in a professional manner. Attends all staff meetings, agency meetings, in-services, and clinical trainings as instructed by supervisor. Maintains confidentiality on all cases at all times. Follows goals of the program and policies of United Counseling Service. 4.0 COMMUNICATION: Ensures that all persons involved with provision of services and support to the client receives appropriate communication about the care and planning related to the consumer. * In addition, performs other duties as assigned. Qualifications: EDUCATIONAL/EXPERIENCE/CREDENTIAL REQUIREMENTS: Preferred Bachelor's degree in Social Work, Psychology, or related Human Services field.
