Description
The LPN Case Manager coordinates services through an interdisciplinary process to provide a clinical and psychosocial approach through the continuum of care. Assists in development, planning, coordination and administration of the activities of clinical review, discharge planning, resource utilization and utilization review.
Graduate of a practical nursing program. Three (3) years of clinical nursing experience in an acute care setting is strongly preferred. Case management and/or Utilization Review experience also preferred. Current Mississippi LPN licensure is required. BLS certification is preferred.
