Job Type: Contract to Hire
Location: St. Cloud, Florida 34769
Hours: 8:00 AM - 5:00 PM (Monday - Friday)
Salary: Competitive and Dependent on Experience
Job Responsibilities:

- Provide comprehensive patient care.
- Manage hospital updates and discharges.
- Address transition of care needs and schedule accordingly.
- Coordinate with hospice and VITAS services.
- Track and follow up on hospitalizations.
- Triage patient needs effectively.
- Ensure seamless communication between patients and healthcare providers.
- Maintain accurate and timely documentation.
- Registered Nurse License
- Case management experience
- Strong healthcare background
Preferred Qualifications:
- Bilingual
