**Job Title:** Registered Nurse **Job Specialty:** Discharge Planner/Case Manager **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 16:30) **Guaranteed Hours:** Not specified **Experience:** Minimum 2 years' experience required; must have 1 prior travel assignment **License:** Must have a DC License in hand, social workers must be eligible for LMSW/LGSW/LCSW and have passed the exam **Certifications:** BLS, RN or LMSW/LGSW/LCSW **Must-Have:** Experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters **Job Description:** - Work setting skills required: Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing - Work setting skills preferred: Home Health, Hospice, telephonic, previous charge experience - Case Management/Utilization Review skills required: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Workers' comp case management - Regulatory Skills Required: Knowledge of CMS, Department of Health, HIPAA, The Joint Commission/Core Measure/National Safety Goals - Regulatory Skills Preferred: Experience with Workers' Compensation - Patient Diagnosis exposure: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU - Work every 5th to 6th weekend as needed - Uniform requirement: Business Casual; appearance should be clean, neat, and professional. No scrubs. - Flu vaccine is mandatory with no exemptions. **Additional Notes:** - Hours spent on modules are billable up to 4 hours; any additional hours for modules are non-billable.
