About The Position
**Job Title:** Clinical Case Manager **Job Specialty:** Hospital Case Manager **Job Duration:** 36 weeks **Shift:** 9-5, Monday to Friday **Guaranteed Hours:** 40 hours per week **Experience:** Minimum 5 years of experience in clinical nursing. 1-2 years in case management or a related field preferred. **License:** Current licensure to practice in the state of California **Certifications:** Basic Life Support (BLS) required, Advanced Cardiovascular Life Support (ACLS) preferred **Must-Have:** - Ability to multitask - Proficiency in using computers and various software systems including Microsoft Office, Athena, EMR, and Point Click Care - Ability to work effectively in a fast-paced environment - Knowledge of community resources and support services - Bilingual in Spanish preferred **Job Description:** - Plan, organize, and coordinate swing bed admissions and review emergency department admissions and discharges. - Provide administrative and clerical services and contribute to process improvement. - Coordinate with the referral specialist to manage, obtain, and share medical records. - Work under the general supervision of the Director of Nursing. - Perform financial assessments to determine patients' source of payment for in-patient stays and ensure timely payment for hospital services. - Provide information in response to queries from doctors' offices, families, and outside facilities. - Manage case management, social services, utilization review, and discharge planning on a daily basis, including weekends as scheduled. - Identify and coordinate swing admissions, verifying that admission requirements are met before transfer. - Review emergency department admissions and discharges, contacting patients to arrange follow-up visits or coordinate transfers as necessary. - Develop and maintain relationships with specialty offices to facilitate transfers or collaboration in care. - Participate and collaborate with the interdisciplinary team and attend Utilization Review meetings. - Review patient charts for counseling requests and assess the need for intervention or appropriate community resources. - Be available at least two Saturdays a month, depending on admissions. - Educate patients and families about the disease process and treatment. - Connect, inform, and advise all ancillary departments of any changes to the patient care plan.
