Summary:
MCIL is seeking a Case Manager to coordinate, implement and monitor comprehensive consumer-directed systems of support and advocacy for persons with disabilities and their families through implementation of case management for effective community integration.
The ideal candidate will be self-directed and have strong problem solving skills. The successful Case Manager will be able to prioritize work, work independently as well as with a team, and will have working knowledge of community resources such as MA, Public Assistance, and Public Housing. This position will be responsible for providing community based referrals and working with consumers to achieve self-direction and community integration.
Duties:
- Provide client-centered case management services that assist individuals who receive Medical Assistance and Home and Community Based Waiver services in locating and gaining access to needed waiver and other state plan services as well as needed medical, social, housing, supportive and other services.*
- Coordinate client intakes, discharges, team and individual meetings and services within set parameters and timelines.*
- Meet in client's homes while traveling through Designated County and surrounding areas*
- Identify and explore service options to meet clients' assessed needs and make referrals to outside resources based on assessment, current client need and individualized plan.*
- Evaluate and monitor services identified in the service plan.*
- Administer a comprehensive assessment yearly or as needed of clients' healthcare, social needs, housing, educational, vocational and other service needs and preferences.*
- Monitor services provided to the client to evaluate the effectiveness and assure safety. *
- Monitor changes in regulations in case management and waiver funding and modify services to meet those changes.*
- Ensure client right to privacy, compliance with HIPAA and all county and state regulations.*
- Maintain billable time and the necessary daily records for continuity of services and reports to funding agencies. Accurately document services delivered and timely submit billing to Case Management Program Director.*
- Monitor waiver budgets and waiver allocations to ensure appropriate service utilization. Report any over or under spending issues promptly to Case Management Program Manager.*
