Job Summary: Under the supervision of the Operations Director, oversees and manages the daily operations of the St. Paul’s PACE program including operations of the Day Center, Clinic, Social Work, Rehabilitation, Recreation, Home Care, Transportation, Dietary and Medical Records. This includes ensuring that appropriate personnel are trained and perform their functions within the organization. Duties include assuring that operational services standards and budgetary criteria are met. Additionally ensures operations are in compliance with all governing regulations. Collaborates with other St. Paul’s PACE sites to ensure standard practices across the organization and lead Quality Improvement initiatives. To ensure continuity of operations, assumes the Operations Director’s responsibilities in her/his absence.
Qualifications and Requirements:
Education: Bachelor’s Degree in business administration, or a health or human services field, or RN degree/diploma. Shall be at least 18 years of age.
Experience: A Minimum of five years of health care experience, and three years of management experience, preferably in a geriatric care setting required. A minimum of one years experience in working with the frail or elderly population. Shall have either training or related experience in the job assigned. The Center Director shall be knowledgeable of the physical, mental, and social needs of the elderly and be a professional in one of the following: Nursing, Social Work, Psychology, Recreation, Occupational Therapy, Physical Therapy, Speech Therapy, Dietetics, or Gerontology.
Skills and Knowledge:
- Working knowledge of California State and Federal PACE regulations.
- Working knowledge of San Diego’s healthcare and geriatric services networks.
- Demonstrates effective leadership and decision-making skills.
- Demonstrates effective supervisory skills.
- Ability to foster collaborative working relationships.
- Possesses strong group facilitation skills.
- Working knowledge of physical, mental, and social needs of the frail elderly and their families.
- Working knowledge of health care quality improvement and cost containment systems.
- Ability to apply creative problem-solving skills to complex issues.
- Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.
- Grasp of the knowledge base from which various disciplines assess the needs of the elderly population.
- Effective written and oral communication skills.
- Working knowledge of spreadsheet, database, and/or word processing
Physical Demands and Working Conditions:
- Physical Requirements – Shall be in good mental and physical health, and capable of performing assigned tasks. Requires standing, walking, occasional pushing and pulling, and lifting. Requires the ability to lift up to 50 pounds. Requires manual and finger dexterity and eye-hand coordination.
- Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, caregivers, participants, and community at large.
- Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments. Subject to people that may have the potential for verbal or physical aggression.
- Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
- Attends in-service education program, including orientation, skill training and continuing education.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Completes training in first aid and in cardiopulmonary resuscitation and maintains certification.
- At assigned Center, directs, supervises, and evaluates the performance of the Day Center Supervisor, Clinic Supervisor, Social Work Supervisor, Rehabilitation Supervisor, Medical Records Coordinator and Technician, Administrative Assistant for Center Director, and Transportation Coordinator. Shares supervision and performance evaluations with peer Center Director(s) of Home Care Coordinator and Dietary Supervisor.
- Ensures that the job responsibilities, authorities and accountability of all direct reports are defined and understood. Provides regular monitoring and feedback to ensure optimal performance of direct reports.
- Ensures compliance of staff to all SPSS policies, guidelines and professional standards, etiquette, and codes of conduct.
- Provides Center operational oversight and designates on-site operational oversight to assigned supervisory level direct report when out of the Center for St Paul’s related business or approved time off. Informs Operations Director of oversight plan when out of the building during Center hours of operation.
- Is a member of the St. Paul’s PACE Interdisciplinary Team (IDT) and acts as facilitator of IDT and care plan meetings. Ensures service delivery requests and authorization for services are approved with team member consensus and are medically necessary within program scope of services.
- Coordinates the Interdisciplinary team’s care management process to ensure best decisions are made regarding the participant’s change of condition.
- Ensures the coordination with home health, rehabilitative, hospital, long-term care, end of life and medical /specialty services according to participant care plans.
- Ensures approval of durable medical equipment for participants is appropriate and accurately accounted for.
- Ensures required staff annual trainings and competencies per regulation and/or policies are conducted in a timely manner and documented.
- Provides or delegates in-service education to staff as identified and needed.
- Continuously evaluates the participants' changing needs and makes necessary program improvements. Develops, expands and modifies the program in accordance with the needs of the participants served. Collaborates with Operations Director and peer Center Director(s) for best practices, care coordination and standardization across PACE sites.
- Routinely attends in person and is an active participant in leadership meetings including Clinical Leadership Team, PACE Leadership Team, SPSS Safety and Operations Business meetings to develop centralized standards.
- Engages direct reports in documentation expectations and standards including but not limited to health record documentation, notices of action, disenrollment's, grievance resolutions, internal encounter reporting, health and safety denials.
- Assists in the development and implementation of the St. Paul’s PACE Annual Quality Improvement (QI) Plan. Facilitates department annual goal development, tracking and monitoring as part of Annual QI plan.
- Participates regularly in quality assurance/quality improvement committees as part of the overall quality improvement program.
- Addresses participant complaints and/or grievances with professionalism and in a timely manner. Assists with resolution or corrective action plan with department supervisor and Quality Assurance Manager.
- In conjunction with the Operations Director and Compliance Officer, maintains the stability and reputation of St. Paul’s PACE by insuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing St. Paul’s PACE operations.
- Assists in development and directs implementation of St. Paul’s PACE operational policies, procedures, protocols and regulations for direct participant care and support services.
- Regularly informs Operations Director of participant, operational or safety issues and provides reporting accountability as directed.
- Monitors operations to ensure consistent compliance with established quality assurance standards, policies and regulations.
- Monitors St. Paul’s PACE operational compliance with OSHA regulations and Center safety policies and procedures. Develops self audit plan for ongoing regulation, policy and internal standards compliance.
- Performs safety and emergency preparedness drills according the Emergency Preparedness Plan. Performs functions of incident commander in actual emergencies and directs staff according to safety plan and policies to ensure safety of participants, staff and any persons within the Center.
- Collaborates with Enrollment Supervisor and Operations Director to meet site specific goals for target enrollment. Ensures timely Level of Care assessments (Initial and Annual) for submission to the State. Ensures that Assessments contain sufficient data from members of the Interdisciplinary Team (IDT) to clearly identify any need for PACE services. Reviews and approves with clinical leadership team all IDT recommended Health and Safety denials prior to submittal to State reviewers.
- Recommends changes and efficiencies in service delivery and staffing as required by the development and growth of the Center.
- Plans for Alternative Care Site programming and utilization to support Center growth. Ensures service delivery at Alternative Care Sites conforms to participant care plan and care is coordinated with Center.
- Works with supportive housing manager to ensure participant care is coordinated and provided according to Supportive Housing MOU/Contracts.
- Establishes positive working relationships with health, social services and contracted providers. Collaborates with corporate Contract Manager to address contracted provider issues, related program orientation and problem resolution.
- Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
- Provides assigned Center participant on-call coverage on a rotation basis and provides back up administrative support to on call staff as needed.
- In collaboration with the Executive Director and Operations Director, assists with evaluation of St. Paul’s PACE operational results against established service standards, and budget and other financial criteria. Assists in development and implementation of performance improvement plans as indicated by evaluation of it.
- Upon formal completion of St. Paul’s PACE budget competency training, and in collaboration with the Executive Director and Operations Director, assists in development of annual operational and capital budgets for departments of accountability. Ensures financial benchmarks are met to ensure continuity of operations, assumes the Operations Director’s responsibilities in her/his absence.
