About PACE
Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

One Team One Mission
Position Summary:
The Certified Occupational Therapy Assistant is responsible for providing clinical occupational therapy assessment and treatment for program participants, on acute, restorative or maintenance levels as needed by day center, nursing home or in-home setting. Integrates the occupational therapy treatment plan into team plan of care (POC) under the direct or indirect supervision of the Occupational Therapist following the plan of care for maximum holistic gain for participant. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
Essential Functions:
• Provides clinical occupational therapy assessment/treatment on acute, restorative and maintenance levels to program participants based on the occupational therapist assessment and treatment plan.
• Administers treatment program per OTR instructions and treatment plans in addition to clarifying physician's treatment instructions
• Confers with interdisciplinary team on participants care issues and follows up with participants during treatment, explaining precautions to take
• Discusses progress of participants plan of treatment and recommendations with interdisciplinary team condition and treatment plans and recommends solutions to participants care problems with interdisciplinary team
• Liaison with GMWP's nursing home and assisted living staff regarding occupational therapy needs of participants who are their residents
• Assessments of charts and reports on participant's progress
• Assists in recommending occupational therapy policies and procedures concerning services provided
• Maintains quality assurance program, safety, environmental and infection control under the guidance of the occupational therapist
• Educates participants and family members in proper care and use of various supports and exercise programs and use of durable medical equipment at home according to care plan.
• Trains family members, other caregivers, and GMWP staff in methods to maximize safety and functional abilities in performing ADLs based on occupational therapist assessments
• Fulfills GMWP documentation requirements and department record-keeping of occupational therapy services provided
• Other duties as requested by the Rehabilitation Manager

Education and Certification:
• Graduate of an Occupational Therapy Assistant program is required
• 2-year Associates of AS curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) or other equivalent organizations is preferred
• Eligible for the National Registration Examination of the National Board of Certification for Occupational Therapy Assistant.
• Must have a current driver's license/Auto insurance and clean driving record must be in compliance with Company policy.
Skills & Experience:
• A minimum of one-year experience working with the frail elderly population is required
• A minimum of two years of health care experience with nursing home and home health experience including home visits preferred
• Achievement of a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service
• Working knowledge of the scopes or practices of physical therapy and speech/language pathology related to geriatrics
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment. COVID-19 vaccination is required for all staff.
