Client Directed Services Program Lead (Independence)
The CDS Program Lead assists persons with physical, mental, or multiple disabilities in accessing consumer-directed personal attendant services. The CDS Program Lead also provides case management services, information, referral services, and performs record-keeping functions as well as providing training to all consumers and attendants as needed. The Program Lead is expected to represent the company in a professional and courteous manner at all times.

Responsibilities
- Assists in evaluation of consumers for program eligibility and referral to State.
- Responsible for documenting services provided according to policy and procedures.
- Maintains clear, concise and accurate records in the CDS program.
- Responsible for keeping accurate records of community contacts.
- Maintain records of completed requests for information and/or Referrals for services.
- Makes frequent home visits to consumers in several counties.
- Perform consumer/attendant file audits regularly.
- Complete monthly reports in a timely manner for CDS team as required
- Maintain case management notes and activity schedules.
- Provide Consumers and attendants with appropriate training material as needed.
- Marketing of program in coverage areas for your office.
- Performs other duties deemed necessary by In-Home Manager/Executive Director.
- Provide/maintain 4 core services assessment tools per month.
Requirements
- Bilingual (Spanish) is a plus!
- Supervisor Experience Preferred but not required
- Knowledge of office management systems and procedures
- Proficiency in MS Office (MS Excel and MS PowerPoint, in particular)
- Excellent time management skills and the ability to prioritize work
- Attention to detail and problem solving skills
- Excellent written and verbal communication skills
- Strong organizational skills with the ability to multi-task
- High School degree or equivalent
- Will train the right candidate
