About the Job
As a Care Coordinator Assistant, you will work from a team approach, and in continuous partnership with patients and clinicians, to provide administrative and diversified support to the Care Team (clinician, medical assistant, nurse, etc.) by coordinating care for patients. Pay range starts at $18.15 and placement in the range is based on evaluation of qualifications and professional experience. Schedule is Monday through Friday.
In this role you will:
- Support the team by scheduling appointments by incoming and outgoing calls.
- Gather and locate patient records, receive faxes and distribute to assigned staff.
- Serve as an effective communication link between patient and clinic staff by gathering information from patients.
- Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
- Schedule patients for preventive care needs.
Requirements:
- High school diploma or equivalent required.
- Minimum of two years of experience in a health care environment providing direct administrative patient care strongly preferred.
- Prefer those with Epic EHR experience.
Pay Range:
$17.80 - $24.91

PTO and Personal Time accruals are pro-rated by FTE/hours worked. Non-benefits eligible employees will accrue Personal Time based on hours worked and State worked.
