- Functions to serve as an advocate, educator and coordinator of services for at-risk patients.
- DEPARTMENT DESCRIPTION
- The Samaritan InterCommunity Health Clinic is a comprehensive, patient-centered primary care clinic for IHN-CCO members with the goal of promoting equitable and culturally humble care for all, by aligning the health plan with the clinical team to increase access, improve quality outcomes, reduce cost, and enhance coordination of care.
- EXPERIENCE/EDUCATION/QUALIFICATIONS
- High school diploma or equivalent required.

One (1) of the following required:
- Bachelor's degree in a health related field and one (1) year experience in a healthcare setting.
- Associate degree or successful completion of a technical program, plus a Registered or Certified Medical Assistant and two (2) years’ experience in a healthcare setting.
- Three (3) years experience in a healthcare setting and two (2) years professional customer service experience.
- Healthcare Provider Level BLS required within 90 days of hire.
- Certification or Registration as a Medical Assistant preferred.

Experience in the following preferred:
- Outpatient, ambulatory care and/or community healthcare.
- Care coordination.
- A primary care related setting.
- Chronic conditions.
- Electronic medical information systems.
- Bilingual preferred.

- Team Building - Ability to work, function and communicate on a multi-disciplinary team. Possess the knowledge and skills to develop constructive and cooperative working relationships with others and maintain them over time.

Basic understanding of age-related differences in caring for and/or communicating with patients and caregivers. Possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles, and ages.
- Service Orientation - Successfully apply knowledge of various resources/models of recovery to assist and care for others.
- Clerical - Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, designing forms, and other office procedures and terminology.
- Confidentiality - Knowledge of State and Federal (HIPAA) laws pertaining to confidentiality of protected health information. Ability to comply with laws and maintain confidentiality of patient information.
- Time Management - Ability to organize, plan and prioritize work to complete within required time frames and to follow-up on pending issues. Ability to perform multiple tasks simultaneously and thrive in dynamic fast-paced environments and under pressure.
