Pay Range: $85,000 - $100,000

About the Role:
The RN Clinical Care Coordinator plays a key role in helping ALWP clients receive the services they need through the Care Coordination Agency (CCA). You will lead efforts to assess client needs, create individualized care plans, and monitor ongoing service delivery to promote positive health outcomes. Additionally, you will serve as a vital clinical resource for the Community Care Transition (CCT) program, helping patients transition smoothly into supportive care environments.
What You’ll Do:
• Coordinate and monitor care for waiver recipients, ensuring access to state plan services, community resources, and healthcare providers.
• Conduct in-person assessments using the ALWP tool to determine the appropriate level of care.
• Develop and update Individualized Service Plans (ISPs) to reflect client needs.
• Meet with clients monthly (in-person and virtual) to track progress and adjust care plans as needed.
• Maintain accurate clinical documentation, including charting, progress notes, and case records.
• Collaborate with Residential Care Facilities (RCFEs), Skilled Nursing Facilities (SNFs), and healthcare providers to support clients’ care plans.
• Investigate and report any signs of abuse, neglect, or fraud in accordance with state guidelines.
• Participate in training, quality improvement activities, and compliance initiatives.
• Travel across Los Angeles, San Bernardino, San Diego, Ventura, and Fresno Counties to meet clients and attend conferences.
What We’re Looking For:
• Registered Nurse (RN) License: Valid and in good standing with the State of California.
• CPR Certification: Current certification required.
• Experience:
• At least 2 years in home health, primary care, or healthcare facilities.
• 1 year of experience in healthcare quality management (preferred).
• Familiarity with the Nursing Practice Act and California’s Assisted Living Waiver and Community Care Transition programs.
• Understanding of Title 22 regulations and mandated reporting processes.
• Technical Skills: Strong clinical, documentation, and analytical abilities.
• Communication: Excellent verbal and written communication skills in English.
• Mobility: Licensed driver with access to a reliable, insured vehicle.
• Health Clearance: Clear TB screening or chest X-ray and pre-employment physical.
