Program Description
The Reentry Case Management team offers mental health services to TAY and adults with mild to moderate mental health conditions and includes a continuum of care, including treatment of dual diagnosis and justice system-impacted individuals. This program will help increase coordination among its service delivery systems; support clients in navigating transition of care; expand the inter-displinary treatment team approach of planning of care, treatment, supervision and services for individuals; and improve client outcomes to reduce recidivism among residents receiving supervision under AB 109 (realignment).
Overview of Position
• Provide case management, mental health and substance use recovery and wellness services, and crisis prevention and intervention within context of multi-disciplinary team
• Develop and maintain good clinical relationships with clients
• Utilize evidence-informed and evidence-based interventions to address client needs and goals
• Collaborate with community partners acting as current access and intervention points; including jails, Adult Probation, and community social service providers
• Meet documentation and other deadlines; comply with requirements for Medi-Cal/county paperwork.
• Perform administrative tasks within timelines (chart completion, meeting attendance, etc.)
Required Qualifications
• A minimum of 2 years of case management and mental health treatment for TAY and adults who are justice impacted (including experience with co-occuring disorders)
• Masters degree or higher in Social Work, Counseling, Psychology or related clinical field
• Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skills
• Ability to work cooperatively on a multi-disciplinary team, and to work with clients with various ethnic backgrounds
• Knowledge of DSM 5
• Willingness to work from a strength-based, harm reduction, client centered model with an emphasis on evidence-based practices

Work Schedule: Onsite
FLSA Status: Exempt
Salary Range: $85,000-$90,000 Annually (DOE)
