Founded in 1970, RHD serves tens of thousands of people every year with caring and effective programs: addressing intellectual and developmental disabilities, behavioral health, homelessness, addiction recovery and more.
Program Summary:
The Pennsylvania Institute for Community Living (PICL) is a 15-bed modified therapeutic community residential program within RHD that offers integrated self-help, recovery-oriented treatment along with psychiatric rehabilitation, 24-hour staffing and placement in permanent housing. PICL offers all components of Psychiatric Rehabilitation in its day-to-day treatment with specific lesson plans and groups organized around functional skill sets in the following areas: social skills, vocational skills, tenancy skill building such as budgeting and financial planning, time management, emotional expression and coping skills. The program usually takes 12-18 months for residents to reach their recovery focused goals.
Position Summary:
This role is responsible for organizing patient care activities and sharing information among all the participants concerned with a patient’s care to achieve safer and more effective treatment outcomes. A commitment to the RHD values should be demonstrated as job duties are performed. The rate for this position is: $22.20 - 23.62 per hour, which is approximately $43,290.76 - $46,054.00/year.
Essential Duties and Functions:
- Program Development and Monitoring.
- Be familiar with all program funder requirements, available community support services, government licensing agencies and other related resources.
- Build and enhance relationship with program participant, RHD program staff and outside providers to coordinate care.
- Maintain up to date knowledge and services provided throughout the RHD service line.
- Through regular and systematic review, ensure thorough analysis of each client’s care and ensure that they are linked with all available resources.
- Develop comprehensive treatment plans for each client that needs two or more services across settings.
- Review compliance of individual treatment plans.
- Maintain tracking system to identify need for comprehensive treatment plans.
- Interface with in-house and external partner providers through monthly/quarterly off-site visits.
- Facilitate routine comprehensive treatment team collaborative planning meetings.
- Provide statistics and outcome/measures on quality of care.
- Update/respond to weekly and monthly reporting requirements.
Qualifications:
- Master’s Degree in the Human Service field.
- One to Two years’ experience working with children and/or adults with Mental Health Diagnosis/IDD/Drug and Alcohol Treatment.
- Valid Driver’s License.

- Direct Supervisor: Unit Director.
