VNA of Ohio is looking for a Clinical Liaison (RN or LPN) to join a dynamic team. The position is responsible to drive liaison activities and clinical training to hospice referral sources and sales representatives in a specified market area to achieve sales goals through effective business development within specified focused accounts. Works with physicians and other health professionals to complete evaluations and consultations, including obtaining necessary information to complete plan of care and referral coordination. Position reports to the Executive Director of Strategy.
Responsibilities:Illustrative duties may include, but are not restricted to, one or more of the following:
- Receives orders, coordinates home care and completes patient assessments/communication, and obtains all needed forms to complete a referral.
- Works closely with personnel in skilled nursing facilities, physician’s offices, home care coordinators and discharge planners in the community and with the VNA Intake staff, to coordinate a smooth, accurate and complete submission of referral information.
- Ability to effectively present features and benefits of all VNA service lines.
- Communication with accounts through consistent feedback and follow up regarding
patient status using available clinical data and key referral source data/reports. - Identifies opportunity for community educational programming, presentations, health
fairs, CEU’s and collaborates with Community Education Specialist to coordinate activity - Commitment to meeting monthly budgeted numbers of referrals, preparing and
submitting activity plans, statistics and reports electronically, as directed - Perform other related duties based on departmental need.
Qualifications:
Qualifications and Requirements:
- Completion of an accredited nursing education program with a Diploma, Associate or Bachelor's degree or other healthcare related training/education preferred.
- Five (5) years of clinical experience in community, hospice and home health care.
- Communicating with people outside the organization, representing the organization to customers, the public and other external sources. This information can be exchanged in person, in writing, or by telephone or e-mail.
- Knowledge of medical insurance, regulations and COP’s for certified home health care.
- Sales, marketing, and advertising and/or community education experience preferred.
- Proficient in Microsoft Office, including Word, Power Point and Excel to create weekly and monthly reports.
Required Licenses, Registrations and/or Certifications:
- Nursing certification; licensed in the State of Ohio.
- CPR certification.
- Valid driver license
