Responsibilities:
ESSENTIAL DUTIES:
• Models a service attitude towards others, takes ownership in solving problems and
takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
• Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
• Maintain good attendance record
• Provide for optimum access to hospice programs and services by responding to
referrals in a timely manner through concerned and compassionate listening and assessment skills
• Facilitate access to hospice services and programs that best meet the patient/family need
• Present a calm, supportive approach to patients/families to facilitate two-way communication and maximize their ability to deal with end of life issues and decision- making
• Maintain close collaboration with Social Services/Case Management, physicians, and hospital nursing employees on referred patients/existing hospice patients who are hospitalized
• Perform initial assessments with chart review; Collaborate with nursing employees and physicians regarding diagnosis, prognosis, and symptom management issues
• Provide information and education regarding programs and services available to patient/family
• Speak with referred/existing patients/families regarding individual needs and goals; Provide daily support to patient/family, answering any/all questions and providing feedback
• Determine eligibility/continued eligibility for the hospice general inpatient or alternative program
• Keep accurate, concise documentation with attention to detail and follow-up as instructed per hospice policy/regulatory policy
• Complete all necessary documentation for admission to General Inpatient
• Assess need for appropriate intervention and provide these services in conjunction with the hospice/hospital employees
• Assist with implementation or ongoing revisions to the patient/family plan of care
• Assist Hospice Medical Director/hospital physician with pain and symptom management by providing frequent feedback on the patient’s response to treatment
• Assist in coordinating discharge planning or bereavement follow up
• Attend and participate in IDT meetings; Attend staff meetings as scheduling permits
• Participate in departmental planning and the development of team objectives under the strategic plan
• Complete all mandatory licensure requirements
• Perform other duties as assigned by the Director of Business Development.
MINIMUM QUALIFICATIONS:
• Valid Texas RN License
• At least three years of nursing experience; hospice experience preferred
• Knowledge of hospice programs and services
• Excellent physical assessment skills and the ability to assess patient needs, counsel patients/families and admit to the appropriate program
• Ability to educate/collaborate with other teammates and coordinate services in a hospital setting
• Extensive knowledge of reimbursement issues and community resources
• Ability to perform Essential Duties as outlined below
• Genuine caring for and interest in the care of mature adults, handicapped people and hospice Patients
• Ability to perform work tasks within the Physical Demand Requirements as outlined below
• Ability to comply with the Dying Persons Bill of Rights and Employee Responsibilities
• Ability to comply with organizational and departmental safety policies and procedures
