POSITION SUMMARY:
The Clinical Liaison provides for a standard of excellence in nursing practice through the provision of direct care, consultation, education and management leadership services for patients, families and the rehabilitation team. The following general duties and responsibilities are essential elements for employment at the facility. In addition to the duties and responsibilities that are specific to this position, the following represent minimum criteria with which one must comply.
RESPONSIBILITIES:
· Provides quality care for the patient and acts as a role model for other nursing staff.
· Works in conjunction with physicians and the rehabilitative team to coordinate and deliver rehabilitative services to all subacute and residential patients.
· Interfaces nursing care standards of practice with the interdisciplinary team model.
· Assists in the development, implementation and evaluation of nursing care plans and interdisciplinary care plans via the MDS 2 Plus format.
· Serves as a resource for nurses at all levels and for other health care providers for advanced clinical practice in a specific area of expertise.
· Practices standard precautions at all times. Follow specific precautions when applicable.
· Evaluates the clinical outcomes of nursing practices, resource utilization and environmental conditions.
· Does all the admissions to the subacute unit.
· Demonstrates effective decision making for efficient matching of demands and resources to resolve clinical and professional issues.
· Consults with all nursing staff on issues related to clinical problem solving and nursing care planning.
· Consults with members of the substance team on issues related to medical and rehabilitative care.
· Consults with patients, families, and potential patients on issues related to subacute care. Visits referral sources as appropriate.
· Contributes to nursing knowledge through scholarly publications and professional presentations on clinical topics and issues covered in nursing.
· Assesses learning needs and designs, implements and evaluates comprehensive teaching programs for specific patient populations, health care providers and community groups to improve patient outcomes.
· Facilitates the acquisition and application of clinical knowledge, theoretical knowledge and decisions making skills by nurses and other health care providers.
· All other duties as assigned by supervisor.
EDUCATION:
· Registered Professional Nurse licensed in the state of MA.
· Graduated from an accredited master’s degree in nursing.
· Minimum five years of experience in acute care nursing.
· Minimum two years of experience in staff development/nursing education.
· Awareness of current trends in nursing, standards of care and legal issues.
