We are hiring a Clinical Liaison to support our admissions team on the weekends. This is a great opportunity for a NURSE, PT, OT, OR SOCIAL WORKER who is looking for a part time, weekend only position. ONLY CANDIDATES WITH A CLINICAL LICENSE WILL BE CONSIDERED. Non-licensed candidates will automattically not qualify.
The coverage for this role is for four(4) of the Landmark skilled nursing facilities (SNF's), but is a remote position.
The four (4) facilities for which the Clinical Liaison will provide weekend coverage are:
- Casa de Ramana in Framingham.
- The Neville Center in Cambridge.
- St. Joseph Rehab and Nursing Center in Dorchester.
- Saint Francis Rehab and Nursing in Worcester.
Some of the responsibilities of the Clinical Liaison include:
- Establish a strong marketing presence in targeted hospitals and referral sources within the Center’s geographic area. Cultivate new referral sources, maintain current referral relationships, and take a lead role in the marketing activities for the Center.
- Facilitate the flow of referrals of patients and admissions from acute care provider(s) by fostering relationships between patients, physicians, and/or case managers.
- Assesses patients, review their medical records, and determine whether they should be admitted to the Center for care.
- Explains the types of care and services the Center offers to patients and their families.
- Coordinate with the Center staff to ensure smooth patient admission.
- Verify eligibility for care under their health insurance and its coverage, interacting with the patients, family members, HMO Case Managers, the Center’s Business Office Managers and Admissions Directors.
- Establishes and maintains positive relationships in the community through activities, events, symposiums, and other platforms such as media, radio, and on-line.
- Meet and exceed the budget for the census and quality mix for the Center.
Qualifications of the Clinical Liaison:
- Five years minimum clinical nursing or therapy experience, preferably in long term care.
- Experience in sales or business development, basic understanding of commercial insurance, Medicare and Medicaid reimbursement.
- Proficiency with running reports and using the data to establish trends, outcomes and strategies for relationship management.
- Strong computer skills and experience with electronic medical records.
