Full-time days consisting of being in the office in Farmington with the occasional home visit needed to lead the team towards excellence in home care.

Position Summary:
RNClinical Manager is a qualified full-time employee appointed by the GoverningBody to direct and supervise professional and auxiliary personnel providinghome health services. The RN ClinicalManager will be responsible for the direction, coordination, and supervision ofhome health services.
Essential Functions:

- Follow policies and standards established by theGoverning Body, and under the supervision of the Administrator.
- Assist with planning, coordination, leading, andevaluation of home health services.
- Responsible for coordination of care with allagency departments, clinical team, committee’s referral sources, and communitypartners.
- Consults, plans, and works with Administrator inpolicy and program development.
Duties/ Responsibilities:
- Consults with leadership and staff, regarding servicesprovided to patients.
- Evaluates the services provided and clinicalteam.
- Lead and coordinate Weekly Case Conference Meeting
- Schedule – weekly nurse schedule; monthly oncall schedule; input on call scheduled into on call service portal
- Manage Clinical System - Point Click Care (PCC)Dashboards
- Monitor / complete coding portal and QualityAssurance process
- Directs the implementation of the individualizedplan of care, including goals and coordination of services.
- Develops standards to ensure safe and effectiveservices are provided to patients.
- Establishes and maintains effective channels ofcommunication with patients, the patient’s responsible party, leadership,clinical team, referral sources, and community partners.
- Ensures staff development, includingorientation, in-service education, continuing education, and competencyevaluation.
- Directing and monitoring performance improvementactivities, including Quality Assessment and Performance Improvement(QAPI).
- Complete patient visits as needed
- Assist with staff orientation and training
- Follows and implements policies and procedures.
- Ensures policies and procedures are maintainedand updated per regulations.
- Participates in staff meetings and committees,as indicated per leadership team.
- Follows, implements, and updates appropriate agencydocuments, leadership, and agency personnel regarding federal, state, and localregulatory practices.
- Ensure standards of ethical business andclinical practice are maintained.

- Completes other duties as assigned by theleadership team.
Qualifications/Education/Experience/Skills
Education:
- Must be a Registered Nurse.
- Minimum of one year of home health experience.
- Must have at least one year of supervisory oradministrative experience in home health or related health programs.
- Must meet initial and ongoing educationalrequirements as specified by the Federal and State regulation.
Licenses/Certifications :
- Current state Registered Nursing (RN) license
- Must meet initial and ongoing educationalrequirements as by the State.
