The Embedded Clinical Pharmacist is responsible for the daily activities of the pharmacy programs. Assists with the development and execution of numerous initiatives that improve the quality or cost effectiveness of drug therapy in coordination with other care team members and physicians as identified.
Core Responsibilities/Essential Functions
- Provide direct face-to-face or telephonic pharmaceutical care services to ambulatory care patients in a clinical setting and promote appropriate, safe and cost-effective drug therapy to improve patients' quality of life.
- Perform patient medication therapy monitoring and disease state management counseling.
- Document pharmaceutical interventions within the electronic medical record.
- Assist patients and physicians with medication profiles and facilitate prior authorization as related to patient and point of care need. Maintain significant physician engagement and support within an assigned geographic area.
- Assist in the development and presentation of educational materials for patients and providers.
- Respond to phone inquiries (including but not limited to drug information, eligibility, health plan navigation) for both internal (Markets, Specialists, Medical Directors) and external (Pharmacists, Physicians, Patients) customers. Serve as a drug information resource to the clinical team, assigned prescribers, and patients.
- Proactively conduct analysis of care setting drug utilization data to identify trends, develop strategies, and implement comprehensive programs to improve patient outcomes.
- Responsible for utilizing available data to implement health plan pharmacy initiatives, such as HEDIS/Star measures and Medication Therapy Management Program (MTMP).
- Utilize Pharmacy Benefit Manager resources and maintain comprehensive knowledge base on all heath plan pharmacy benefits.
- Serve in the capacity of clinical preceptor to provide oversight of students and/or residents to execute specific outreach programs including HEDIS/Star measures, MTMP, and other initiatives.
- Participate in patient rounds as a member of the multidisciplinary care team where applicable.
- This role requires at least 25% travel within the Huntsville, AL area.
Minimum Qualifications
• PharmD or Bachelor's degree in Pharmacy with advanced degree
• Registered Pharmacist in state of AL
• Three or more years of pharmacy experience
• Knowledge of computer applications, including pharmacy claims systems
• Knowledge of regulatory requirements with emphasis on Medicare
Preferred Requirements
• Managed care pharmacy experiential rotation or residency
. Two or more years of clinical and/or managed care experience
Must reside near the Huntsville, AL area. Role will require local travel.

For this position, we anticipate offering an annual salary of 111,900 - 186,500 USD / yearly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
