Responsibilities
- Client-Centered Services: Provide individualized services as identified in treatment, wellness, and safety plans, including specialized services for women. Facilitate individual, group, and family therapy, along with skills and educational groups, ensuring a minimum service level is maintained.
- Collaborative Scheduling: Work with supervisors and other clinicians to create a well-structured client service schedule that meets funder requirements and addresses individual client needs.
- Timely Documentation: Complete required services, data entry, and paperwork, including assessments, treatment/recovery plans, discharge summaries, and progress notes, within specified timeframes (e.g., assessments and plans within 24 hours; progress notes within 48 hours).
- Promote Recovery Model: Advocate for harm reduction, trauma-informed care, and self-directed recovery by utilizing recovery language in documentation and interactions.
- Holistic Referrals: Provide referrals, linkage, and advocacy to community resources and services that address clients' mental health, substance use, and primary care needs.
- Community Engagement: Foster positive interactions within Chestnut and with external stakeholders, including family members, probation officers, DCFS case workers, and court personnel, to maintain a favorable program reputation.
- Professional Development: Attend all meetings and trainings and meet ICB’s GCE requirements within two years of hire.
- Skill Building: Demonstrate proficiency in evidence-based practices such as Community Reinforcement Approach (CRA) and Motivational Intervening, with a focus on women-specific EBPs.
- Customer Service Excellence: Adhere to Chestnut’s behavioral standards to promote a culture of customer service excellence.
- Confidentiality: Maintain and protect the confidentiality of all organizational information, including financial and consumer data.
- Additional Duties: Perform other duties as assigned or negotiated with your supervisor.
Qualifications
- High school diploma or equivalent and five years of human/social services; OR
- Bachelor’s degree;
- Certified Alcohol and Drug Abuse Counselor (CADC) certification AND Certification Board’s Gender Competency Endorsement (ICB GCE) withing two years of hire
- Basic computer skills and experience with MS Word, MS Excel, Outlook, and electronic medical records (EMR).
