The Community Account Executive work is accountable for enrolling eligible patients into the HarmonyCares program. This position will lead community outreach activities for the team and collaborate to build relationships with physician offices and healthcare facilities with the goal of engaging shared patients into the Harmony Cares program.
Essential Duties and Responsibilities
- Be accountable for the community outreach account plan for assigned geography which may include one or multiple markets
- Conduct thorough research on community provider practices, organizational structure to drive account plans
- Conduct thorough research to understand patient's healthcare needs, healthcare utilization and best number to call
- Outreach and conduct in-person visits to physician offices, hospitals and skilled nursing facilities to educate community providers and engage patients into the HarmonyCares program
- Collect, analyze and report patient outcomes and experiences to community physicians
- Conduct telephonic outreach to eligible patients or caregivers to enroll patients in the HarmonyCares program to support team engagement goals
- Understand the program in depth, explain it to patients, caregivers and providers and handle objections creatively
- Be the face of HarmonyCares in the community and collaborate closely with physician leaders to support community relationships
- Leverage electronic tools, spreadsheets and reports to facilitate research, identify innovative approaches, and document and present plans and progress
- Meet or exceed established monthly engagement targets
Required Knowledge, Skills and Experience
- College degree or 5 years of progressive career growth
- 3+ years of related experience in sales, account management, community outreach or patient engagement in a medical/healthcare industry
- An understanding of value-based care and how it can impact patient outcomes
- Proficiency in Microsoft Excel, Word, and PowerPoint, ability to generate Excel reports
- A dynamic and engaging personality with excellent interpersonal, verbal and written communication skills
- Detail oriented, with the ability to think on your feet, responsive with great follow-up
- Compassion, an interest in people and a professional presentation
- Curious and a continuous learner, a passion for solving problems with a get-it-done attitude
Preferred Knowledge, Skills and Experience
- Bilingual in Spanish, Mandarin or other language a big plus
- Comfort with CRM and EMR tools a plus
