Full time Community Care Coordinator in our Reseda office
Hospice Referrals and Intake experience preferred.
Mon-Fri 8am-5pm
Hourly rate depends on experience, range is $23-25/Hour

POSITION SUMMARY:
The Community CareCoordinator (CCC) serves as a conduit between our clients and their businessdevelopment staff, clinical staff and community referrals. The CCC is a clerical position with administrativeand client promotional responsibilities.
QUALIFICATIONS:
- Education/Credentials: Preferred - Bachelor’s degree or healthcare/clinical setting experience.
- Experience: Atleast two years of administrative or sales experience.
- Core Competencies: Knowledgeof medical terminology and regulations regarding medical records,confidentiality and HIPAA. Demonstrates excellentoral and written communication skills and attention to detail.
- Other: Validdriver's license, and auto insurance, reliable/well maintained transportation.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
1. Assists with placement,admission process and any other assigned tasks associated with service topatients and families.
2. Updatesreferrals, placement locations and others with status reports
3. Reports to management on various clinical andadministrative matters.
4. Maintainscontinuing education through academic studies, seminars, and workshops.
5. Attends andparticipates in in-services, staff meetings and case conferences.
6. Completesall documentation in an accurate and legible manner.
7. Complies with companyand clients’ policies and procedures, laws and regulations and standards ofpractice.
8. Other tasks,duties, projects and responsibilities as may be assigned.
