Position Summary | The University of New Mexico isseeking a Lead Community Health Worker to join the Office for Community HealthAdult Inpatient Social Determinants of Health Team. The Office for CommunityHealth is looking for a full-time Lead Community Health Worker to work withadults during their inpatient stay at The University of New Mexico Hospital tohelp screen for and provide support for Social Determinates of Health.

Minimum Qualifications | High school diploma or GED; at least 5 years of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. |
Preferred Qualifications | * Experience working with clients living with special needs * Experience in providing comprehensive case management * Experience in organizing, understanding, and connectingclients to resources * Experienced providing Community Health Worker services tomedically and socially complex populations * Experienced working with the LGBTQ+ community * Experienced working in a multi-disciplinary setting * May be required to obtain a Community Health Worker (CHW)certification within six months of hire. |
