Position: Program Coordinator - Community Outreach
Work Location: Sunset Park
Hours: Full Time - Monday - Friday 35 hours per week Ability to work some evening and weekend hours required
Salary Range: 60,000-62,400
Program Summary:
This program supports unhoused individuals living on the streets by addressing their basic needs and providing essential services, linkages, and referrals. Additionally, it offers safer sex education and substance use support and information.
Position Summary:
Under the supervision of the Program Director, the Program Coordinator will oversee Community Health Workers and a Peer Trainer as part of a comprehensive outreach program. This program delivers education, testing, counseling, showers, and referrals to unhoused individuals living on the streets in high-risk communities, focusing on preventing HIV, Hepatitis C, and substance use.
Responsibilities:

Provide consistent supervision and management for all aspects of high quality program services delivery to clients that are unhoused and living on the streets in accordance with funder and regulatory requirements; best practices; BCS policies and Core Values. Serve as a role model to all stakeholders.

Provide weekly supervision to CHWs and Peer trainer

Work collaboratively with the Program Director to submit monthly and quarterly reports to funders.

Will work closely with drop in centers, safe haven, substance abuse detox and rehabilitation programs and medical providers to link clients to services while providing training education and HIV testing for the unhoused population.

Ensure that Community Health Workers are providing HIV and Hepatitis C Testing, education and training to individuals living on the streets.

Ensure that CHWs are distributing high-risk prevention supplies, including female & male condoms, safer hygiene kits, first aid, opioid overdose prevention, fentanyl testing strip kits, and provision of sterile syringes to targeted populations..

Help people understand their at risk behaviors and develop strategies to improve their health and well-being through HIV and Hep. C testing, education supportive counseling and follow-up

Over see that all data is being input in programmatic databases (AIRS, Apricot, etc)

Coordinate community-based events including outreach and education (e.g., program brochures, tabling, Naloxone training and distribution, community wellness activities)

Build community trust through service-based work and relationship-building

Deliver health information using culturally appropriate terms and concepts

Ensure that CHWs are linking people to health care and social service resources including health insurance, food, housing, quality care and health information.

Facilitate groups and workshops

Follow organization’s protocols for safety and confidentiality guidelines.

Participate in Team and departmental meetings

Completing required number of annual training hours.
Qualifications:

Bachelor’s degree with at least one year of health/social services experience or Associates degree with three years health/social services experience

Supervisory and management skills required

Valid Driver’s license

Preferred certification to perform HIV and Hepatitis C testing and counseling

Demonstrated ability to manage time and projects and submit work in a complete, on time and accurate manner.

Interpersonal skills necessary for teamwork, decision-making and management and to work collaboratively and effectively within a team.

Demonstrated ability to respond with sensitivity and confidentiality in compliance with HIPAA and other regulations.

Demonstrated ability to effectively work with people from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.

Familiarity with Motivational Interviewing approach helpful
