The Community/Provider Liaison (NJ Pediatric Psychiatry Collaborative (NJPPC)) provides consultative support to pediatricians and pediatric providers throughout NJ to assist them in managing their patients behavioral and mental health needs within their medical practices. Some of the services that the NJPPC provides are informal consultations to pediatricians by our child and adolescent psychiatrists, formal consultations to assist pediatricians in bridging medication to patients, and care coordination to pediatricians patients in need of mental health resources. The NJPPC seeks a full time community/pediatrician liaison to join a growing team and be an active member in the recruitment of pediatric providers to participate in the NJPPC as well as establish relationships with community agencies to make them aware of the NJPPC.
Responsibilities:A day in the life of a Community/Provider Liaison at Hackensack Meridian Health includes:
1. Identify and recruit pediatricians/pediatric groups and K-12 Schools to participate in the statewide Pediatric Psychiatry Collaborative program.
2. Build and retain relationships with providers, educating them about targeted programs and services provided through the PPC program.
3. Establish and maintain ongoing lines of communication among community pediatricians, K-12 Schools, community agencies, and Statewide Pediatric Psychiatry Collaborative program.
4. Assist in development of monthly schedules of outreach activities based on strategic and programmatic priorities of the department.
5. Accumulate feedback and comments through outreach visits to identify needs, problems and requested information.
6. Provide ongoing support for relationships established with registered pediatricians and serve as the point person when communicating any new information and areas of concern.
7. Identify and network with community-based resources that could be used to facilitate care coordination
8. Identify and implement ways for the PPC program to improve referral processes and consultation services by reporting and tracking pediatrician feedback.
9. Compile information to disseminate to community pediatricians (i.e. CME programs, update on new faculty/services, newsletters,etc.)
10. Meet regularly with site leadership to obtain a strong understanding of site objectives and how to help meet these goals.
11. Coordinate and collaborate with program managers to schedule orientation sessions with identified pediatricians requesting to join the PPC.
12. Collect and maintain a database of all pediatricians and K-12 schools who are enrolled in the NJPPC program and other data as needed.
13. Other duties and/or projects as assigned by PPC leadership.
14. Adheres to HMH Organizational competencies and standards of behavior.
Education, Knowledge, Skills and Abilities Required:
1. Bachelor's degree in business, marketing or healthcare.
2. Experience in the mental healthcare industry.
3. Ability and willingness to travel extensively throughout the state on average 80% of the time required.
4. Ability to work at a fast pace, prioritize multiple activities and respond in a timely manner to requests.
5. Self-motivated, adaptable, flexible, and ability to network effectively with others.
6. Adapt hours and availability to meet the needs of our physician customers.
Education, Knowledge, Skills and Abilities Preferred:
1. MBA
2. Three (3) years in the healthcare and/or service industry which includes some clinical, marketing, and sales experience.
3. Knowledge of medical, behavioral health, and psychiatric terminology.
