Optima is seeking a medical assistant for our Care Management department. This individual will be expected to monitor patients’ aftercare and support them in achieving short/long term health goals. The medical assistant will coordinate with providers to ensure patients’ needs are met. A working knowledge of the clinical field is desired for this position.
Care Manager Responsibilities:
- Manage a roster of patients who have enrolled in the remote patient monitoring program.
- Have a full understanding of each patients individual care plan.
- Record patient data daily and alert healthcare providers of abnormal reports.
Care Manager Qualifications:
- At least 1 year of clinical work experience as a Medical Assistant
- At least 1 year of EMR experience; eClinicalWorks preferred
- High level of professionalism
- Excellent written and verbal communication skills
- Strong customer service skills
- Medical Assistant Course Completion Diploma or CMA designation required
