SUMMARY OF POSITION:
The Coordinator of Case Management will oversee Case Managers in assuring their job duties are completed in accordance with agency and Dept. of Aging & Independent Living (DAIL) / Centers for Medicaid Service guidelines and/or regulations. The coordinator may maintain a small caseload and have knowledge and experience with all Case Manager functions. Directly responsible to the CDS Assistant Program Director. Experience with person-centered planning, quality assurance, development, and monitoring of the Individual Support Agreement (ISA) and comfort with providing training. 40 hours/week with benefits
SPECIFIC DUTIES AND RESPONSIBILITIES:
- Clinical supervision as well as collaborative problem-solving consultation to Case Managers and team members as appropriate. This may include specific Case Manager difficulties, as well as case updates.
- Coordinate coverage for services during Case Manager’s time off (e.g. vacations and sick time).
- Interview, and facilitate the hiring process and training of new Case Managers when necessary.
- Liaison on behalf of your team with quality control assurance personnel (monitors) to meet billing requirements. Oversight responsibility for record compliance by Case Managers to meet DAIL requirements.
- Review ISAs and other mandatory documents, provide technical assistance to improve and expand quality of paperwork for case managers.
- Familiarity of UEMR, and able to provide training and assistance with the Electronic Medical Record.
- Facilitate team meetings to ensure the efficient delivery of services; disseminate administrative information to team members; and perform other logistical duties as needed.
- Provide direct supervision of Case Managers.
- Participate and ensure that clinical oversight as indicated by consumer need (ISA) is followed by Case Manager and team.
- Assume Case Manager role when Case Manager is unavailable or arrange for back up.
- Ensure that personnel issues are addressed in a timely manner collaboratively with HRD, Case Manager, APD, and PD.
- Participate/attend leadership meetings as required.
- Monitor with Case Managers, the funding level of each consumer to assure adequate resources to meet priority needs of the consumer. Assure fiscal resources are utilized or returned as appropriates.
- On-call coverage on a rotating basis
- Other duties as assigned.
QUALIFICATIONS: Bachelor’s degree, supervisory experience, and Qualified Developmental Disability Professional (QDDP) status required. Possession of a driver’s license, excellent driving record, and access to a safe, reliable, insured vehicle are required.
