Day Center Coordinator Job Highlights:
Under the supervision of the Center Manager, manages the daily operations of the Senior CommUnity Care Adult Day Health Center in conformance with Center policies, procedures, and standards and all governing state and federal regulations. Responsibilities include the planning, coordination and implementation of all activities in the Day Health Center as well as the supervision of the Day Center Workers.

- Bachelor's degree or equivalent education/experience.
- A minimum of one (1) year of experience in a geriatric health setting.
- Proficiency with various business software, preferably Microsoft Office.
- Must have a valid driver's license, proof of insurance and have means of transportation.
- Ability to foster collaborative working relationships. Strong interpersonal and group facilitation skills and ability to manage multiple teams. Ability to supervise, direct, coach, and develop direct reports and larger team. Demonstrated ability to work with a diverse team and flourish in a flexible work environment.
- Must have medical clearance for communicable diseases and up-to-date immunization before having direct participant contact.
- Master's Degree in health care management/administration, or nonprofit/public administration preferred.
- Knowledge of physical, mental, and social needs of the frail elderly and their families
Essentials:
- In consultation with the Center Manager and other management staff, develops and implements policies, procedures and standards for the Adult Day Health Center. Communicates changes to staff, monitors outcomes and makes changes as needed.
- Interviews and recommends for hire, staff reporting directly to the Day Center Service Supervisor.
- Provides for orientation, ongoing training and competency evaluation of staff according to program needs, and state and federal regulatory requirements. Ensures that the job responsibilities, authorities and accountabilities of all direct reports are defined and understood.
- Fosters team-building, professional relationships and good communication among staff and other departments. Routinely communicates and implements changes in program goals, philosophy and policy to staff.
- Identifies the continuing education needs of assigned staff, and collaborates with the Center Manager and Clinical Services to establish the most effective and efficient approaches to addressing same.
- Assists in the development of organizational staffing patterns and position descriptions for assigned staff.
- Collaborates with Home Care Supervisor, Clinic Supervisor and Center Manager to coordinate home care services.
- In consultation/coordination with other directors and managers, oversees and manages the operation of the Senior Community Care (SCC) PACE centers to provide a safe and productive environment that is conducive to the effective and efficient delivery of PACE services
- In conjunction with the Center Manager, is responsible for maintaining compliance with all State and Federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
- As a member of the Interdisciplinary Team, actively communicates relevant issues to the team and ensuring all team members are informed and up to date; acts as facilitator, when designated, of Interdisciplinary Team Plan of Care meetings.
- Maintains the Day Health Center and its equipment as required by plant operations and state and federal agencies.
- Monitors the physical environment of the Adult Day Health Center to ensure that a safe, orderly, and sanitary environment is consistently maintained for participants and staff. Reviews safety issues, including participant falls and incidents as they occur, and reports to the Center Manager and appropriate committees.
- Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
- Fulfills all administrative requirements, such as development of Day Center Workers schedule, oversight and completion of daily-required documentation and reports associated with State and Federal agencies.
- Establishes positive working relationships with health and social service providers. Interfaces with participants, families, contracted providers and the public in regard to issues related to the routine operation of the Day Center.
- Acts within the scope of his or her authority to practice
- Monitors Senior CommUnity Care's operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.
- Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs related duties as required or requested.
