Direct Care Specialist - Group Home / Adult Day Program - The Center for Health Care Services

POSITION SUMMARY: The Direct Care Specialist will provide routine care for consumers, and complete associated documentation for the consumer and program. There are several positions and several shifts avaialble with the opportunity to work at multiple group homes.

1 FT Seasonal in the Youth Camp Program, 2 FT Direct Care Specialist @ Calidad the Adult Day Program, 3 FT Direct Care Specialist @ Calm Lake and Quivira Group Home, 3 Part-time positions in the Group Homes.

ESSENTIAL FUNCTIONS &ndash: PRIMARY RESPONSIBILITIES AND DUTIES

• Provide personal care services (assistance with activities of daily living and independent living skills)
• Assist the activities supervisor with recreational activities (n/a for some programs)
• Provide protective supervision (observation and monitoring)
• Assists with Ambulation and mobility (Assistance may range from total physical assistance with or without adaptive equipment, to verbal support)
• Ensures clinical documentation is submitted in accordance with Center standards and meets all designated requirements
• Assist with consumer assessments, and with development of goals and objectives

Required Experience:

Minimum requirements

• High School Diploma or GED
• Six months of experience in human service care, preferably individuals with intellectual developmental disabilities
• Experience with operating telephone, fax machine, copy machine, calculator, motor vehicles: (lifting devices and other physical therapy equipment may be required for some programs)
