Job Title: Community Health Coordinator
Responsibilities:
- Coordinate health services for members within the Medicaid program.
- Facilitate communication between members and healthcare providers.
- Assist in the development and implementation of health education programs.
- Monitor member health outcomes and report findings to management.
- Support outreach initiatives to increase member engagement.
Qualifications:
- Bachelor's degree in Public Health, Social Work, or related field.
- Experience in healthcare or community services preferred.
- Strong communication and organizational skills.
- Ability to work collaboratively in a team environment.
