Job Title: Community Health Coordinator

Responsibilities:

• Coordinate health services for members within the Medicaid program.
• Facilitate communication between members and healthcare providers.
• Assist in the development and implementation of health education programs.
• Monitor member health outcomes and report findings to management.
• Support outreach initiatives to increase member engagement.

Qualifications:

• Bachelor's degree in Public Health, Social Work, or related field.
• Experience in healthcare or community services preferred.
• Strong communication and organizational skills.
• Ability to work collaboratively in a team environment.
